The Lingering Impact of Crisis on Hawaii’s Mental Health

A study highlighted a tragic 46% increase in suicide and overdose deaths across Hawaii following the fires, with an alarming 97% spike on Maui alone. For many survivors, the path to recovery is long. Recent findings show that half of adult survivors screen positive for depressive symptoms, and nearly 50% of affected children show signs of post-traumatic stress disorder (PTSD). This prolonged distress is made worse by “survivor burnout,” as many have become tired of retelling their traumatic stories to outside organizations, feeling their voices are not truly heard.
These events have not affected everyone equally. They have magnified existing disparities, placing an even heavier burden on already vulnerable populations.
If you or someone you know is struggling, help is available. Call the Suicide Prevention Hotline at 988 or text 988 for confidential support 24/7.
Disparities in Care for Native Hawaiian and
Pacific Islander Communities
For Native Hawaiian and Pacific Islander (NHPI) communities, the path to mental wellness is often blocked by generations of systemic barriers. Historical trauma from colonization, combined with racism and cultural stigma, has created significant health inequities that persist today. These deep-rooted issues contribute to poorer mental health outcomes and a reluctance to seek help.
Cultural expectations of strength and resilience can make it difficult for individuals to admit they are struggling. Within NHPI communities, seeking help for mental health can be viewed as a sign of weakness, leading many to suffer in silence. This is compounded by a healthcare system that often lacks culturally competent providers who understand the unique experiences of these communities.
Statistics paint a clear picture of this disparity. Older NHPI adults are two to three times more likely to experience depression compared to other populations. Despite this clear need, NHPI adults are three times less likely to receive mental health services than non-Hispanic whites. This gap highlights a need for accessible, culturally sensitive care that respects and incorporates community values.
Systemic Barriers to Mental Health Care in Hawaii
Beyond specific events and cultural factors, broader systemic issues make it difficult for many in Hawaii to access the mental health care they need. These challenges affect residents across all islands and from all walks of life.
The High Cost of Care
One of the most significant barriers is financial. The high cost of treatment prevents many people from seeking help, even if they have insurance. Co-pays, deductibles, and out-of-pocket expenses for therapy and psychiatric services can be significantly more expensive than for other types of medical care, placing essential support out of reach for countless individuals and families.
Shortage of Mental Health Professionals
Hawaii is also facing a major shortage of mental health providers. This scarcity leads to long waiting lists and makes it difficult to find a therapist who is a good fit, especially for those seeking providers from specific cultural or linguistic backgrounds. The lack of culturally aligned professionals further alienates communities that already face barriers to care, making it harder for them to build the trust necessary for effective treatment.
An Ineffective and Fragmented System
A recent report from the University of Hawaii Economic Research Organization (UHERO) concluded that these systemic failures—not just individual choices—are the primary drivers behind the overall decline in mental health across the state. The lack of coordination between primary care and behavioral health services creates a fragmented system that is difficult to navigate, leaving many people without the support they need to thrive.
